Both options give you the same handmade site, built by the same person, from a list of facts you sign off on. The difference is whether the site also answers customer questions for you.

Not included. An Agent that answers customer questions or takes bookings. That is the second option, opposite.

What moves the price: how many questions the Agent has to handle, whether you want bookings or intake forms, and how busy the shop is. Year 1 covers building the Agent and the site together. Year 2+ is ongoing relationship, not maintenance.
